[Abstract] Emotion regulation (ER) has been implicated in the development of psychopathology and is a key target in psychological interventions. This systematic review aimed to determine whether ER mediates treatment outcomes and to examine how these effects vary by intervention type, ER construct, outcome domain, and study design. Eligible studies were peer-reviewed randomized controlled trials that tested ER as a mediator of treatment effects on mental health or behavioral outcomes, using a formal mediation analysis. We searched Medline, Web of Science, and PsycINFO on March 29, 2025. Sixty-two studies (61 articles) met the inclusion criteria, yielding 202 unique mediation analyses. Most studies included adult participants from Western countries. Across all analyses, 50% showed significant indirect effects of treatment via ER. Stronger mediation effects were observed for interventions explicitly targeting ER—especially those grounded in established ER models. Studies using passive control conditions reported a greater proportion of significant mediation effects than those with active controls. ER appears to mediate treatment outcomes in a substantial number of trials, particularly when it is a core target of the intervention. However, considerable heterogeneity in measurement, intervention design, and methodological rigor limits strong causal conclusions. These findings support ER as a potential transdiagnostic mechanism of change but highlight the need for more robust and diverse designs to clarify its role across contexts.
