【摘要】引言:性相关问题在人的一生中普遍存在。因此,医学生及其他卫生专业人员接受充分的性医学教育至关重要,这将使他们能够在日常执业中妥善处理此类问题。目的:整合并更新既往“国际性医学咨询会议”发布的建议,以及全球范围内关于医学生性健康教育(SHE)标准的其他共识性建议;评估针对各类卫生专业人员的建议现状,以及现有的培训与教育平台情况。方法:对既往关于医学院校性健康教育课程标准的报告进行概念性综述,并对2016年至今的相关文献进行更新综述。结果:基于专家意见、既往经验及文献综述,我们针对医学生性健康课程提出了总结性建议和综合性指导意见(包括教学方式等),同时考虑到全球课程设置的多样性。目前关于性医学培训对临床技能持久性及患者预后影响的研究尚显不足。现有的性医学教育平台可在不同培训层级、学科及地域间进行应用、调整与优化。我们探讨了改进知识与胜任力评估的机遇,以及研究改进课程与评估方法的重要性,特别是针对此前尚未开展此类课程评估的学科。结论:鼓励性医学学术组织落实既往建议,并将医学生性健康教育列为重点工作。将这些建议推广至研究生教育、执业临床医生及其他卫生专业人员的教育中,有助于弥合教育与临床实践之间的差距。未来的培训干预措施应纳入对技能掌握情况及有意义的患者预后的评估,并整合各类教育平台,包括网络形式、交互式技术及体验式学习策略。

[Abstract] Introduction: Sexual problems are common throughout the lifespan. Therefore, it is important for medical students and other health professionals to receive instruction in sexual medicine that is sufficient to prepare them to address these issues as a routine part of practice. Objectives: To consolidate and update recommendations published from prior International Consultations on Sexual Medicine and other consensus-based recommendations from across the world for standards of sexual health education (SHE) among medical students. To evaluate the status of recommendations for various health professionals and of existing training and educational platforms. Methods: A conceptual review of previous reports on curriculum standards of SHE in medical schools and an updated literature review from 2016 to current was conducted. Results: We provide a summary and comprehensive recommendations based on expert opinion, accumulated experience, and literature review for a sexual health curriculum for medical students, including how it should be taught, recognizing that curricula are diverse worldwide. Scant research exists regarding the impact of sexual medicine training on sustained clinical skills and patient outcomes. Existing sexual medicine education platforms can be applied, adapted, and optimized across training levels, disciplines, and geographic locations. We discuss opportunities to improve evaluation of knowledge and competency, as well as the importance of studying methodologies to improve curriculum and methods of evaluation, particularly for disciplines in which these curricula have not previously been evaluated. Conclusions: Sexual medicine societies are encouraged to implement previous recommendations and make medical student SHE a priority. Extending these recommendations to post-graduate and practicing clinician and other health professionals’ education would help close education and clinical practice gaps. Future training interventions should incorporate assessment of skill acquisition and meaningful patient outcomes, as well as integrate educational platforms including web-based formats, interactive technology, and experiential learning strategies.
